*Please bring all cats in a carrier when visiting our veterinary hospital or you will be charged $6.00 for a cardboard box carrier to ensure the safety of your cat.*
Please choose the forms that are appropriate for your situation.
NOTE: All forms have been provided as .pdf documents.

which is FREE to download.
Spay & Neuter Forms:
-Print and fill out the Surgery Authorization Form and a Spay or Neuter form for EACH cat.
1)  Surgery Authorization Form
2) FEMALE: Spay Form or 2) MALE: Neuter Form
Vaccine/Other Clinic Service forms:
-Print and fill out the Registration Form and a Vaccine & Basic Services Formfor EACH cat.

Prepare Your Cat's Surgery:
- Remove all food and water bowls at midnight the night before his/her surgery in order to avoid conflicts during and after surgery.
- Drop off your cat IN A SECURED CARRIER between 8AM-9AM on your appointment date. Preferrably 1 cat per carrier, unless they're small kittens. Ferals in traps if possible.
- Pick up 3PM-4PM the same day.
